5 YEARS ON THE JOB · CONSTRUCTION

Sarah is onboarding a new subcontractor crew arriving Monday morning. She has 30 minutes before the morning safety meeting. The old orientation is a 47-page PDF nobody has updated since 2022.

Sarah prints 8 copies of the binder, hands them out, runs through the highlights verbally, asks the crew to sign the last page, and files the sheet. Twenty minutes later they're on site. A few hours in, a contractor comes up and asks her a question that was covered in the package, but Sarah didn't have time to walk through it and clearly the worker didn't catch it on their own.

Marcus, New Hire · Day 1

JOURNEYMAN PIPEFITTER · FAB SHOP

Marcus arrives at 7:00 AM for his first day. Company policy says new workers complete the orientation package in their first 30 minutes on shift. The supervisor doesn't have time to walk him through it, so Marcus gets handed the binder and a quiet table in the lunchroom.

What actually happened

Marcus flipped through the 38-page binder. He skimmed the chapter on overhead crane operations, skipped most of the lockout-tagout section because the language was dense, and signed the acknowledgement page on the back. The supervisor came back at 7:35, saw the signed page, countersigned it, and pointed Marcus toward his bay. The form went into his file. Everyone moved on.

What happened at 2:14 PM

Marcus needed to move a steel plate from the rack to his work station. He saw the overhead crane sitting idle and decided to use it rather than wait for the forklift operator. He'd watched another worker rig a similar load that morning, so he hooked the chain through a single pick-point at the centre of the plate and signalled the bridge.

The plate rotated mid-lift, slid out of the rigging, and dropped from about eight feet. It landed two feet from another worker's foot. Nobody was hurt. This time.

The orientation package covered the rigging requirement, two pick points minimum, certified rigging only, and the requirement that uncertified workers must request a rigger. Marcus had signed the acknowledgement. He hadn't actually understood it.

The point A signature is not comprehension. The orientation package contained the right information; the delivery method assumed the worker would absorb it on their own. An interactive course wouldn't have let Marcus past that section without answering a knowledge check on it. The information was already there. The delivery is what failed.
